Also, if I receive a W-9 from a vendor where the form is from a few years prior (i.e., not the IRS's updated form), is that okay to accept?
Forms W-9 do not expire. You are required to get new ones only if the vendor tells you the information has changed, or the IRS tells you the information is not accurate by sending a B Notice list (CP2100/A) or proposed penalty notice.
