I received a W-9. Line 1 indicates an LLC business name and line 2 is the DBA.

Q. (cont.) An EIN was given. The tax classification indicates Individual/SP/SMLLC. Is it correct to list the LLC on line 1 with the EIN, or should an individual be listed on line 1 with a SSN or EIN?
