Is a physical signature required on an electronic ACH form?

Q. We are working on updating our ACH form into Fireform: (FireForm allows you to build your own forms for data collection and workflows to manage form review and processing.) Our IT area included the User Confirmation legal language which was on our paper form in the electronic form. There is a check box to click: "I certify information I provided is correct ..." Is a physical signature required on the ACH form or can this check box be considered the signature? A.
