How current should a W-9 be?

Q. What best practice is used to determine how current the W-9 form is supposed to be to be acceptable?
Currently, we accept W-9 forms that are within the current fiscal year.
Is the best practice to require a W-9 form whether a new vendor is being set up or a modification is being made to a current vendor?
Is it best practice to always require a vendor questionnaire (new vendor information form), regardless of the type of vendor?
